New Patient Registration
We are always accepting new patients from the Rushall, Shelfield and Pelsall areas. In order to register with us, please come to either branch and fill out a registration pack. You will then be booked in for a New Patient Health Check with one of our Healthcare Assistants. This is to help us get to know yourself and your past medical history.
All new patients will be asked to provide two forms of identification: a photo ID (passport, drivers license, bus pass), and evidence of your address (utility bill, bank statement, council tax).
If you have been refused access to our surgery list, you will be informed of why.
Please note: once registered it can take some time for your medical notes to be sent to us from your old surgery, so we may not be able to complete certain tasks for you until we have this.

REGISTERING DURING THE COVID-19 PANDEMIC
We have temporarily suspended requiring new patients to come to the surgery to verify their identity. We have also postponed any New Patient Health Checks until further notice. If you wish to register with ourselves, please download the form below, and either fill it in electronically, or, you can print it out and then take pictures of the filled in forms. Please email all completed forms along with two forms of I.D. (photographic and residential) to our email address:
PLEASE DO NOT COME TO THE SURGERY TO HAND IN A REGISTRATION FORM. PELSALL BRANCH IS CLOSED.
