New Patient Registration
All new patients will be asked to attend the surgery to register. You will be asked to complete a registration form, questionnaire and attend for a new patient interview. This will help us to get to know your past medical history.
All new patients will be asked to provide two forms of identification. One must contain photographic evidence and the other must confirm your address. Examples include: passport, driving licence, utility bill, bank statement etc.
It is important that you realise you will not be registered until your health check has been completed.
If you are refused access to the list you will be informed of the reason why.
